| **Confidential: we do obtain your personal identifying information, but do not disclose that information to any other party |
| ***Anonymous: we do not obtain your personal identifying information at all |
| ****Express Test: tester comes to you within one hour (expect call back within 30 min) |
| *Night Hours: any tests done between 9pm and 6am |
| Surcharge: an extra $50 for any tests done 35-60 miles outside of testers area (> 60 miles, negotiable) |
